A 60 year old patient, who had undergone choledochal cyst excision and Roux-en-Y Hepaticojejunostomy (HJ) in 2016, was now diagnosed with narrowing at previous HJ site with stones in bile ducts above that. He underwent uneventful surgery (stone clearance with wide redo HJ) and was discharged 3 days after surgery. Treatment options for strictured HJ include surgery & percutaneous transhepatic dilatation. Surgery was chosen in this patient due to anatomic considerations and presence of big and numerous stones in CHD. In experienced hands, redo HJ can be safely done with good outcomes.
