Understanding Neuroendocrine Tumours (NETs)

Neuroendocrine tumors (NETs) are relatively not uncommon tumours that usually occur in the pancreas, gastrointestinal tract, lungs, or other parts of the body. They arise from neuroendocrine cells, which control various hormonal functions. While many NETs grow slowly and remain silent for years, others may be aggressive and can spread quickly. Timely diagnosis and tailored treatment plan help ensuregood health and long-term survival.

Treatment Options for Neuroendocrine Tumours

Depending on patient’s general condition, location of tumour, stage of tumour essentially meaning extent of spread) , treatment may include:

  • Surgical Removal – The most effective treatment for localized NETs.
  • Minimally Invasive Surgery – Laparoscopic or robotic techniques for faster recovery.
  • Liver-directed Therapies – For NETs that spread to the liver.
  • Targeted and Medical Therapies – To control hormone-related symptoms and control tumour growth (essentially preventing progression of disease to some extent)

What our Patient’s has to say

  • My father has been diagnosed with CA esophagus and CA colon. We were advised surgery after the chemo cycles by our treating doctors. We were in dilemma whether to go for the complex Hipec + CRS Surgery suggested. Hence we came for second opinion with Dr. Vivek Sir. He carefully reviewed complete history of my father and did all diligent test required to ensure there are no complications. He gave us the confidence and we went ahead with the surgery with Dr. Vivek Mangla's team instead. It was 10 hours long surgery and was successful. My father is recovering well after surgery. I thank the entire team (Dr. Neha, Dr. Suresh, Dr. Sanjeev) for their expert support and care during the hospitalization. Big shoutout to Dr. Sanjeev Arora Sir, who gave us all the moral support and complete guidance required during the surgery time. We will be debted for lifelong for same. I wholeheartedly recommend Dr. Mangla to anyone seeking a GI surgeon who will provide excellent medical care with a human touch.

  • Dr Vivek Mangla and his team were a big part of an extremely major surgery my mom went through at max vaishali. Her advanced stage ovarian cancer had spread to the colon which was a finding in the surgery itself as it never showed up on any of the pet ct scans. It was dealt with immediately by Dr Mangla and his team with quite a lot of advanced surgical procedures done including colectomy, colo rectal anastomosis, ileostomy creation and all of the cancer was removed from the GI tract. She never had any problems with the ileostomy and it was finally successfully closed after 4 months of the initial surgery. The spread to the GI tract was quite shocking to us but with Dr Mangla’s help, treatment and guidance we were assured of successful results. The surgeon himself is humble, approachable, caring and personally pays attention to each of his patient’s treatment.

  • Dr. Vivek Mangla is Awesome! We were referred to Dr Vivek Mangla from Our Very Good Nephrologist as My Mother had a weight loss of around 15 Kgs in the last 6 Months, Haemoglobin level of 5.5 and after transfusion of 2 Units of PRBC(Refined Blood Transfusion) and PET CT reporting few disturbing facts which were directing towards Colon Cancer and trust me no one is ever mentally prepared when it comes to our near and dear ones. During our first meeting, Dr Mangla said that we will have to do Colonoscopy to find out the real nature of disease whether it's a tumor or a Cancer and next day My Mother's colonoscopy and Biopsy was done which clearly directed towards Colon Cancer. Then I met Dr Vivek Mangla again for further course of action and trust me the genuine concern, the real professionalism and true commitment made me feel that My Mother is in good hands as I was clearly explained that why and how the parts of Colon will be removed, how long the Operation will take place and what are the post operative cares which we will have to do. On the day of operation, when My Mother was being taken, the words don't worry Dinesh and Mata Ji were the words which my ears were longing to listen. After the operation, Dr Vivek Mangla showed us the parts taken out and then I realized how complicated was the surgery. That's when I realized how humble he is even after having tons of knowledge and best hands. There's a saying that A man learns from his own mistakes but a wise man learns for others mistakes. Be wise, Every word written by me here is absolutely true and god forbid if any of your family member is suffering from Colon Cancer then I assure You that he is the best hands in NCR.

  • 🙏 A Heartfelt Note of Gratitude 🙏
On 27.08.25, I underwent major surgery for rectal CA at Max, performed by Dr. Vivek Mangla. It was indeed a very big surgery, but the way Dr. Vivek executed it was nothing short of remarkable. Despite the complexity, the blood loss was limited to just one unit—a true testament to his exceptional skill as a surgeon. Such was his expertise that I was shifted directly to my room without even needing ICU care. The very next day I was able to move, and by the second day, I was already walking. This rapid recovery reflects not only his surgical brilliance but also his meticulous care. Beyond being a super surgeon, Dr. Vivek Mangla is a wonderful human being whose personal touch and compassionate approach uplift a patient’s confidence and immunity multifold. From the bottom of my heart, I want to say millions of thanks to you, Dr. Vivek 🙏. I sincerely pray for your long, healthy life so that you continue to save and protect countless lives with your unmatched expertise. Truly blessed to have been under your care. 🌸

  • We are incredibly grateful to Dr. Vivek Mangla Sir and his exceptional team at Max Vaishali. My brother was diagnosed with portal cavernoma and cholecystitis, and although cholecystectomy was advised, the high bleeding risk due to portal cavernoma made surgery extremely challenging. Dr. Mangla’s expertise and confidence gave us hope. He performed laparoscopic cholecystectomy with remarkable precision and care, managing the complex case with minimal blood loss. My brother was discharged on the third day, recovering smoothly. Throughout our experience, Dr. Mangla and his entire team—including the support staff—were consistently polite, attentive, and reassuring. His skill as a surgeon and his compassionate approach truly stand out. We feel blessed to have been under his care and highly recommend him to anyone seeking expert surgical treatment.

    FAQ's

    Are NETs cancerous?

Yes, Neuroendocrine Tumours (NETs) are a type of cancer. They arise from neuroendocrine cells, which can grow abnormally and can spread to the other parts of body. A major difference is that NETs often grow more slowly than most other cancers.

What symptoms should I look out for?

Symptoms usually vary depending on tumour’s location and whether it produces specific hormones. Common signs include:

    • Abdominal pain or discomfort
    • Unexplained weight loss
    • Persistent diarrhoea
    • Flushing or redness of the skin
    • Wheezing or shortness of breath
    • Fatigue

Quite often, NETs do not cause symptoms and are found incidentally during tests for other conditions more so if there ius no specific hormone production. Such tumours (called non functioning tumours) may attain significant size and may spread without causing any symptoms or sometimes just vague abdominal symptoms.

    How are NETs diagnosed?

Diagnosis usually involves a combination of:

    • Blood & urine tests (to check hormone levels)
    • Imaging scans like CT, MRI, or PET scan (DOTANOC/FDG as indicated)
    • Endoscopy or colonoscopy (for tumours in the digestive tract)
    • Biopsy (to confirm the type of tumour under a microscope)
Is surgery always required for NETs?

Not always. Surgery is often the preferred treatment if the tumor can be safely removed. However, some NETs are treated with other approaches such as targeted therapy, hormone therapy, chemotherapy, or peptide receptor radionuclide therapy (PRRT), depending on the size, spread, and activity of the tumour.

    Are NETs hereditary?

Most NETs are not hereditary. However, in rare cases, they may be linked to inherited genetic conditions such as Multiple Endocrine Neoplasia type 1 (MEN1 syndrome ) or Von Hippel-Lindau (VHL) syndrome. If there’s a family history of endocrine tumours, genetic counselling may be recommended.

Which doctor should I choose for NET treatment?

Treatment for NETs usually requires a multidisciplinary team. The most important specialists include:

    • GI & HPB Surgical Oncologist (if surgery is needed)
    • Oncologist (Cancer Specialist)
    • Gastroenterologist or Endocrinologist (depending on tumour site)

Ideally, one should choose a hospital with a dedicated multidisciplinary oncology team for the best outcomes.
